Maternity Claim
In the event of a potential MATERNITY
CLAIM
In order to make a claim for Maternity
reimbursement
Complete a FijiCare reimbursement claim
form and include:
1. Details such as the full name of
Insured, claimant or dependent whichever is applicable with their EDP or FNPF
number and date of birth.
2. Submit original receipts and invoices to FijiCare.
3. Breakdown on receipts, diagnosis should be shown on the claim form.
4. On receipt of the above, then FijiCare will process the claim further.
