Optical/Dental Claim



In order to make a claim for Optical Care or Dental care

Complete a FijiCare reimbursement claim form and include:

1. The member must provide details such as the full name of Insured, claimant or dependent whichever is applicable with their EDP or FNPF number and date of birth.

2. Submit original receipts and invoices to FijiCare.

3. Breakdown on receipts, diagnosis should be shown.

4. On receipt of the above, then FijiCare will process the claim further.
