In the event of a potential TERM LIFE CLAIM

In order to make a claim for Term Life:

Complete a FijiCare Death claim application form and include:

1. Details such as the full name of Insured, claimant or dependent whichever is applicable with their EDP or FNPF number and date of birth, Occupation, address, duration of illness, treating doctor, details of hospital treatment over the last 12 months.

2. Submit original birth certificate of the Insured.

3. Submit original death certificate of the Insured.

4. On receipt of the above, then FijiCare will process the claim further.
